Care Manager

Senior Concerns is hiring a Care Manager to run the Caregiver Resource Center. The ideal candidate has a masters in social work or related field, and has experience working with older adults and family caregivers.

 

This Care Manager will:

  • Provide Case Consultations for Family Caregivers
  • Facilitate Caregiver Support Groups
  • Manage caregiver grants
  • Provide information and referral to older adults and family caregivers
  • This is a full time position, Monday through Friday, located at Senior Concerns in Thousand Oaks

Caregiver: Adult Day Program

Caregiver: Adult Day Program

We’re looking for a fabulous caregiver for our Adult Day Program in Thousand Oaks. 

Our established Adult Day Care Program is in search of experienced and compassionate caregivers who understand the importance of providing high-quality, person-centered care to meet the needs of our clients. An understanding of dementia care is required. Small groups of clients with multiple staff.

This is a steady, consistent position where you will get to know the clients and their needs. We work as a team to provide fun and enriching activities and programs in a safe and clean environment. This position offers a regular schedule and steady hours, Monday – Friday, from 9:30am – 2:30pm.

Requirements:

  • Current TB Test or X-ray
  • Fingerprint Background Check
  • Photo ID & Social Security Card
  • COVID vaccination
  • Physical and Drug Screening

Additional Requirements:

  • We can provide all the required training including CPR, food handling, and working with people with dementia.

Responsibilities:

  • Lead activities and discussion groups for people with dementia
  • Assist clients with personal care, including transferring and bath rooming, as needed
  • Work in a team based, fast paced and fun environment
  • Ensure highest client safety and well-being

Caregiver Skills:

  • Experience working with the elderly
  • Experience and knowledge working with patients who have memory impairment and physical frailty
  • Ability to problem solve quickly
