• Housing, residential or assisted living referrals
• Transportation resources
• In-home safety and modification review
• Review of short, intermediate, and long term needs and goals
• Care planning, caregiver and family education
• Linkage to community and support resources
• Follow up and on-going reevaluation

Cost:

• Initial brief phone consultation at no charge.
• A range of services are available at $50 per hour.
• Sliding fee available with documented proof of need.
